Is that code for "the gate was hanging on by a thread due to lack of preventative maintenance"?
Perhaps it's cheaper in the long run to let it go to hell and fish a gate out now and again.

I expect the collar strap broke. It happens from time to time and if it ever happened to a lock I was going through I would report it myself but not everyone would notice it. Does seem a bit over the top to lose the gate completely. I expect the next boat along closed the gate not noticing it was blatantly wobbly then when emptying the lock the gate went walkabout. It really is up to boaters to spot failures and report them.

More likely to lose the gate on the way up. I'd have thought.
If the gate gets anywhere near the normal closed position, and the lock is emptied, I'd expect the gate to be held in position by the water pressure. Do we know if the collar strap broke, or was lifted off (Not sure that's possible on K&A locks: depends on collar design.

Lock gate falls off PUBLISHED: WEDNESDAY, 28 MARCH 2018 A LOCK gate on the Kennet & Avon Canal has fallen off and closed the canal.
This is now the end of winter maintenance when all should be well, but a lock gate on Padworth Lock (96) was is such condition that it fell off into the lock, Keith Gudgin tells us,
Top offside This is the top offside gate of the lock, and fell off yesterday, Tuesday, and is a rarity, as we have never been aware of such an occurrence previously.
The first thing is for Canal & River Trust to fish the gate out of the water then attempt to replace it before the waterway can be reopened.
