One of my boats has a Perkins P4 in it. I was told it is ex MoD and I think thats likely as it has a PRM160 box which was not available in the 50s AFAIK and an electric coolant circulation pump.
Anyway it has obviously had some sort of auxiliary equipment attached as there is a drive dog and a clamp for something about the size of a starter motor.
Air compressor maybe?
Will take a photo and add it

The drive dog has paint on so probably not been used for a while I would guess.

Agree on the hydraulic theory. One of my Perkins 6354's had a hydraulic pump in this position to run the winches and steering,. It's basically a reversed PTO position.
